    A larger concern is this “lost in translation” issue. This REIT is acquiring 2 biomedical office buildings that are 96% occupied, with a 15-year term with Vertex Pharmaceuticals Inc. (NASDAQ: VRTX). Vertex is now worth almost $20 billion and is expected to be profitable in 2015. Senior Housing will almost certainly get an earnings boost from this lease, but there is one small problem here – most investors believe that they are investing in a company that makes its bread and butter from senior living facilities. Owning biotech leases of healthy large companies is not a bad business; it just changes the focus of the company because this will add about 20% or so in size.

    Few industries have more surprises and high impact catalysts than biotech. It seems that every other week there are events that cause stocks to rocket higher or plummet to earth, usually from drugs that have positive or negative clinical trial results or from surprise FDA approvals or rejections. In the world of biotech, nothing is certain. At the beginning of each year, I like to take stock from these surprises and try to learn from them as best as I can, and ultimately use past history to help in making future predictions. In January, I made a few predictions on where I thought those surprises may be. With a whirlwind of activity in the sector in January, I thought it would be a good time to review each prediction and provide updates throughout the year. I will discuss each one in a separate article, but let’s get the overview out of the way. My prediction list for 2014 was as follows:

    Vertex (NASDAQ: VRTX  ) is going significantly higher and will be a top mid-cap performer Bristol-Myers Squibb will once again be the top performer in the large cap space Sarepta runs then tanks again on a delayed filing for eteplirsen Innate Pharma would be a big under-the-radar gainer.

    What will Vertex do this year?
    Let’s start with biotech company Vertex. This story hasn’t changed all that much since the beginning of the year. Its stock price in 2014 will primarily depend on the outcomes of the two late stage studies for patients with Cystic Fibrosis. To briefly recap, Vertex has undergone a major restructuring of its focus from hepatitis C to cystic fibrosis over the last two years. The Vertex drug Incivek (for hepatitis c) launched in the second quarter of 2011 and quickly became one of the best launches of any drug in history based on quarter over quarter revenue growth. In just its third quarter on the market, it generated $457 million in revenues. However, its stay as top dog in the hepatitis C space proved to be ephemeral as Gilead’s Soval

    Sovaldi selling way beyond our expectations. The initial prescription data are so massively outperforming even the biggest weeks Incivek ever had, we believe any reasonable comparison between the two drugs would indicate that U.S. sales in 2014, alone, are going to exceed $5 billion. In 4Q11, Vertex (VRTX) recognized $458 million in Incivek sales and WK reported 11,409 NRx. Sovaldi is already pacing more than 2x that volume in 1Q14. Sovaldi also has ~70% price premium over Incivek. There is almost no way barring an extreme reduction in volume over the next few weeks that Gilead won’t hit at least $1 billion in sales in 1Q14 (consensus is at $336 million), and that really only accounts for U.S. sales. While we expect a slower uptake ex-US, we note that historical Incivo sales indicate that there is likely to be an additional $1 billion in Sovaldi sales in 2014 coming from ex-U.S. territories. We believe this volume implies that next year’s sofosbuvir/ledipisvir sales are not only set to be the best launch ever, but exceed the highest single year sales of any pharmaceutical product in history…As a result of our model changes, we are raising our price target to $103 and elevating the stock to Outperform.

    What do HIV, epilepsy and cellular suicide have in common? Vertex Pharmaceuticals  (NASDAQ: VRTX  ) . More specifically, VX-765.

    In a recent report in the journal Science researchers at University of California, San Francisco and the Gladstone Institute of Virology and Immunology found that most CD4 T-cells aren’t killed by the HIV virus directly, but by the body’s cellular “suicide response” via a protein called caspase-1 (or interleukin-1 converting enzyme) and stopping this process prevents the progression of HIV to AIDS. What’s more, the researchers showed that this suicide process is specifically inhibited by Vertex Pharmaceutical’s caspase-1 inhibitor VX-765. If that amalgam of letters and numbers sounds familiar, it’s because back in 2011 VX-765 failed clinical trials to treat epileptics, but was deemed safe for ingestion by humans. The latter part of this early failure is crucial as it suggests VX-765 can start human trials as an HIV therapeutic rather quickly.
